Documentation has to match the real operation.
After the initial mapping we prepare or complete the controller/processor role definition, an Article 28 GDPR agreement, the data flow, retention rules, access management, an incident procedure and materials for the impact assessment. The DPO and DPIA obligations are assessed by scope and risk, not by the project name.

A ten-page shared map of care: contacts and a crisis plan, medication and reminders, communication permissions, powers of attorney and data security. The PDF (in Czech) can be filled in electronically or printed.

Before use: the facility completes the controller, legal bases, recipients, retention periods, any transfers outside the EEA and its own operating rules. Only the blank template is published.
An important legal note
These texts are based on current public law and the described operation of RadimCare. Before any live deployment involving health data, extensive monitoring or a new facility contract, we recommend a review by a lawyer and completion of the DPIA, the processor list and the security documentation.
